Adult Mental Health Services

UNIVERSITY CITY/SOUTHWEST COUNSELING CENTER. Outpatient counseling is provided at two convenient locations, and assists individuals with mental health issues. Services are designed to allow individuals to receive treatment within their communities while working and living at home. Referral to rehabilitation programs is also offered. CHESTNUT PLACE CLUBHOUSE. The Clubhouse is a psychiatric rehabilitation program, based on the Clubhouse model developed by Fountain House in New York City. The program provides a "work-ordered day" and employment opportunities to adults with mental illness. The work-ordered day is divided into work "units" where members use and strengthen their talents as they contribute to the operation of the Clubhouse. The Clubhouse gives members the opportunity to improve their skills, have a place to belong, interact together, and gain employment through the Transition Employment Program, and work towards independence. PEOPLE REDISCOVERING INDEPENDENCE, DIGNITY AND EXCELLENCE (PRIDE). A structured group program for adults is provided on a daily basis, Monday through Friday. The program is designed to meet the needs of adults in the process of stabilization after psychiatric hospitalization, or those who are experiencing an increase of severe psychiatric symptoms or behaviors. The main purpose of the program is to help clients regain stability and reduce symptoms. The Partial Hospital program also helps those who are stable on medications though continuing to experience psychiatric symptoms. We assist consumers in developing community and social supports and in identifying and utilizing coping skills in an effort to transition to a less intensive level of psychiatric care. RESOURCE COORDINATION. The Resource Coordination Unit provides assistance for adults with serious mental illness who have mild to moderate difficulty in social, job-related, or daily living skills. INTENSIVE CASE MANAGEMENT. This program provides individualized in-community support for adults with serious mental illness. Services include 24 hour-a-day, 7 day-a-week crisis response. Intensive Case Management provides case management and crisis intervention services to specified individuals with severe and persistent mental illnesses. The unit uses a community support model for consumer care and assists with medical, financial, and housing needs, as well as mental health issues. MOBILE CRISIS UNIT. The Mobile Crisis Unit is designed to handle psychiatric emergencies for individuals 18 and older and for businesses located in the West Philadelphia area. Services include but are not limited to: on-site evaluations, arranging voluntary and involuntary hospitalization, and crisis intervention. CONTINUITY OF CARE SPECIALIST. This service provides consumers discharged from psychiatric hospitals and their families, with assistance and support accessing the large number of community services available to them. Significant aspects of the work include consulting with in-patient staff and assisting with discharge planning. Heavy emphasis is placed on linking discharged consumers into needed Consortium programs and other programs of the consumer's choice.
